Due July 9th, 2010 - What better way to let your community, your elected officials, and most importantly your health center know just how vital and important they are to you and your family than by creating a video detailing exactly that! Creating a short video is a creative and personal way to advocate on behalf of health centers, share your unique story of what the health center means to you, and have fun along the way.

The goal of the contest is to provide a way for YOU (health center advocates, staff and patients) to tell your story using new media and social media outlets as a way to spread these stories and develop greater awareness of health centers.

To Enter: Individuals should create and submit short videos, no longer than three minutes, that address one or more of the following questions; Why is your health center important to you? How does your health center serve your community? How is your health center celebrating National Health Center Week? What does it mean to you to be a health center advocate?

Grand Prize:
Featured Health Center will receive: 1 Free Registration to the NACHC 2011 Policy & Issues Forum in Washington DC* Video submitter will receive a Flip Ultra Camcorder and be recognized with video highlight aired at the 2011 Policy & Issues Forum.

First and Second Prizes: Submitters of the videos will each receive a Flip Camcorder.

Third and Fourth Place: Submitters of the videos will each receive an ipod shuffle

Eligibility
Only individuals 18 years of age or older are eligible to submit videos for the contest. Exceptions can be made for anyone between the ages of 14 and 18 with signed consent from a parent or legal guardian.
